The answer is: Maude Flanders. Maude Flanders, the wife of Simpsons' neighbor Ned Flanders, was killed off in the 2000 episode "Alone Again, Natura-Diddily." Maude was accidentally struck by a t-shirt shot out of a cannon at the Springfield Speedway and fell from the grandstand to her death. Despite being killed off, Maude has made subsequent appearances both in flashbacks and as a ghost or angel.Source
